OSF Innovation, OSF OnCall, University of Illinois Urbana-Champaign and Spoon River College are building a sustainable model of health care to address health disparities in rural communities.
One in five Americans live in rural communities like Cuba, Illinois, for a variety of reasons, such as family ties, sense of community and cost of living. However, residing in geographically isolated areas can bring challenges to staying healthy and lead to disparities in health outcomes.
A vision for transforming care

Project Cuba - A model community for Rural Digital Health Innovation
The overarching goal of project Cuba is to address health inequities in rural communities.
The beginning step of this project is to collect data on the population of Cuba and discover their health needs.
